I, ____________________________ hereby agree to comply with the following conditions, to be evaluated for adherence on a weekly basis by Staff to allow continuation in the residence and participation of the Dignity House.
- Three (3) months minimum residence at Dignity House.
- Strict adherence to all rules, regulations, and Policies of Dignity House. This includes all rules detailed in the House Rules as well as specific directives by Dignity House Staff.
- Attend weekly house meetings, and five outside AA or NA meetings per week.
- Abide by Dignity House curfews (in house by 11:00 pm weeknights, 12:00 Midnight Friday & Saturday until 6:00 am)
- Obtain work, attend school, go to outpatient therapy, or participate in volunteer work for the duration of residence.
- Must be continually working a 12 Step program of recovery under the guidance of a Sponsor.
